Membership and Enrolment Form

Its easier to do this online but if you prefer to download a form, fill it in and email it back to us, then click the download button below. This PDF has been designed to be filled in using your computer keyboard. The input boxes are visible – you can see what the form is like at the right.

After downloading:

1. Find the form (probably in your downloads folder)
2. Rename it e.g. Fred Smith Membership Form
3. Open the form (requires a PDF reader e.g. Adobe Acrobat Reader)
4. Type in all the relevant boxes.
5. Typing your name in the signature box is accepted as a signature.
6. Save the completed form and email it to us once you have transferred the fees. When transferring to our bank account use your name as a reference.
